NHTD & TBI MEDICAID WAIVER PROGRAMS OVERVIEW

New Yorkers with disabilities and/or seniors can choose to participate in these New York State Home and Community-Based Medicaid Waiver Programs and live safely in their community.

WAIVER PROGRAMS

  • Nursing Home Transition and Diversion (NHTD)
  • Traumatic Brain Injury (TBI)

WAIVER PROGRAM ELIGIBILITY

Individuals seeking to participate in the NYS Home & Community-Based Medicaid Waiver Program must:

  • Be at least 18 years of age.
  • Be an active New York State Medicaid recipient.
  • Have medical documentation of brain injury, and have a physical, medical, or cognitive diagnosis.
  • Be eligible for skilled nursing facility level of care.
  • Choose to live in the community rather than in a skilled nursing facility.
  • Be able to identify a safe living arrangement that will meet their needs.
  • Be capable of living in the community with appropriate supportive services.

SERVICES WE OFFER

The New York State Home and Community-Based Medicaid Waiver Program covers a wide range of services. Optimum Choice Services, Inc. offers three of these services which are detailed below:

    Service Coordination
    A coordinator is chosen by the waiver participant and/or guardian to work with them in identifying and coordinating support and services and developing a written service plan. Click here for more information.

    Independent Living Skills Training
    We provide one-to-one training to improve or maintain your ability to live as independently as possible, focusing on essential community living skills. Click here for more information.

    Community Integration Counseling
    Our counseling service is available for those who are coping with changes in their abilities and skills, as it relates to their disability/diagnosis. Click here for more information.

    Community Transitional Services
    Community Transitional Services (CTS) are individually designed services and project management intended to assist a waiver participant in transitioning from a nursing home to living in the community. This is a one-time service per waiver enrollment which provides funds and/or services. These funds/services allow the waiver participant can have the essential items and services to safely transition into their own home from the nursing home.

    Positive Behavioral Intervention & Support Services
    Positive Behavioral Intervention and Support Services (PBIS) services are provided to participants who have significant behavioral difficulties that jeopardize their ability to remain in the community of choice due to inappropriate responses to events in their environment. The primary goal of PBIS services is to decrease the intensity or frequency of targeted behaviors and to teach more socially appropriate behaviors.
